School-Based Health Care (SBHC) Facts & FAQ

SBHC-kids-chalkboardKenosha Community Health Center (KCHC) and Kenosha Unified School District (KUSD) are collaborating to bring School Based Health Centers (SBHC) to your school. This will provide a way to address the health concerns of our children and to keep them healthy all without causing extended interruptions to learning in the class room.

Below are Frequently Asked Questions (FAQ's) we think will help you understand our services and how to contact us.

What can the SBHC do for my child ?



    The SBHC will be staffed and operated by KCHC and will provide on-site medical services. The SBHC provider will be able to address many health issues including:

    Preventative Care Opportunities

      • Well Child health checks
      • Immunizations
      • Seal A Smile dental sealant program
      • Sports physicals
      • Health Education with child and parent

    Acute Health Care Opportunities

      • Diagnose and treatment
      • Referrals as needed
      • Follow up visits as needed
      • Health Education with child and parent

    Chronic Health Opportunities

      • Monitor and treat medical needs as needed based on condition
      • Health Education with child and parent

    The SBHC will not replace your child's regular health care provider, but they can supplement the services he/she provides. SBHC staff will also help families find a primary health care provider outside of the SBHC if you or your child(ren) do not currently have one. The SBHC will NOT be able to provide emergency services. SBHC does not replace any school health procedures or services.



    Do I need to attend my child's appointments with the SBHC staff?



      You are encouraged to be at the appointments with your child. If you are not able to attend, the staff of the SBHC will communicate with you.



      How do I get my child(ren) enrolled into the SBHC?



      In order for your child(ren) to be seen in the SBHC, a signed parent consent form and your child's health history must ordinarily be on file in the SBHC. You can obtain parent consent and health history forms through your child's school, school nurse, or KCHC. Once parent consent and health history forms are completed and signed, please return them to the school's office.



      Does my family need to have health insurance to use the SBHC?



        Absolutely no child will be turned away due to a family's inability to pay. If you have Medicaid or private health insurance, the SBHC will provide services and send bills to your insurance carrier. You will be billed for your co-pay. If your child is not eligible for Medicaid or private insurance, a sliding fee scale will be used to cover expenses. KCHC will work you through the process of determining your copay. Additionally, if you currently do not have health insurance coverage, the SBHC staff will work with your family to help enroll your child(ren) if eligible.



        How do I make an appointment for my child?



        Walk-in appointments are usually available. If you wish to schedule an appointment, please contact the SBJC For your child(ren)'s first visit, you will be asked to provide:
        Health history
        SBHC Enrollment form if not already on file
        Insurance/Medicaid card if eligible
        Should your child get ill during the school day, your school staff will contact you and coordinate care with the SBHC staff.
